How to Earn Rewards in 2025
L.A. Care Medicare Plus members can earn rewards by completing certain health activities. Already completed your eligible activities this year? You’ll still get your reward!
You can use your rewards to buy things like healthy food, gas, over-the-counter items, and more! To learn more, visit lacare.nationsbenefits.com or call (833) 571-7587 (TTY: 711) from 8:00 a.m. to 8:00 p.m., seven days a week. Help is free and available in your language.

When you complete a health activity, let us know so we can send your reward faster. If you don’t tell us, we’ll wait for your doctor to confirm it. This can take 3 to 6 months.
You don’t have to wait — report your progress and get rewarded right away. This is called an attestation, which means you’re letting us know you did the activity.
To get your reward within 48 hours:

How to Use Your 2025 Rewards with Your Benefits Mastercard® Prepaid Card
Your rewards will be added to your Benefits Mastercard® Prepaid Card when we get the information from your doctor or when you report your activity. To learn how to self-report and get your rewards faster, see Get Your Rewards Faster.
Try to use your rewards in the same year you earn them. Unused rewards will expire on June 30 of the next year.
Getting Started
If you don’t have a Benefits Mastercard® Prepaid Card yet, one will be mailed to you. The card will come with simple steps to activate it.
If you already have a Benefits Mastercard® Prepaid Card, you can use the same card to access your rewards dollars.

Use your card at stores like Albertsons, CVS, Food 4 Less, Ralphs, Walgreens, Walmart, and more. Visit lacare.nationsbenefits.com to find a store near you.
Go to lacare.nationsbenefits.com to order approved items. First-time users must register on the Benefits Pro Portal or app. If you’ve already registered, just log in, pick your items, and check out.
You can place an order by calling (833) 571-7587 (TTY: 711) between 8 a.m. to 8 p.m., seven days a week.
Download the Benefits Pro app to your phone.

Important Information About Your Rewards
Your rewards are added to your Benefits Prepaid Card. You can use them for groceries, gas, and other approved items.
Try to use your rewards in the same year you earn them. Unused rewards will expire on June 30 of the next year. If you leave L.A. Care Health Plan, you will lose any unused rewards.

Using Your Benefits Prepaid Card
