Stay Covered with L.A. Care Medicare Plus (HMO D-SNP)

Important Information About Your Health Care Coverage

To sign up and stay in L.A. Care Medicare Plus (HMO D-SNP), you must:

  • Have Medicare Part A, Part B, and Part D
  • Have full Medi-Cal coverage
  • Be 21 years or older
  • Live in Los Angeles County
  • Be a U.S. citizen or have lawful residency status in the United States

To keep getting your care and benefits without any breaks, you need to stay eligible. Read below to learn how to avoid losing your coverage.

What Happens If You Lose Medi-Cal

If your Medi-Cal coverage is not renewed, you will have a 90-day grace period to reinstate it and retain your L.A. Care Medicare Plus benefits.

After the 90-day grace period ends:

  • You will be disenrolled from L.A. Care Medicare Plus
  • You will lose access to:
    • Extra benefits such as your Allowance card, dental, and vision services
    • L.A. Care Prescription drug coverage
  • You may also incur a late enrollment penalty if you delay obtaining new Medicare drug coverage
     

How to Prevent Losing Medi-Cal Coverage

To Stay Covered:

  • Fill out and return your Medi-Cal renewal form to the Department of Health Care Services (DHCS) by the due date
  • Send any extra paperwork they ask for
  • Don’t wait — turning things in late could mean losing your coverage

If You Move

Your home address must be in Los Angeles County. If you moved or got a letter asking to check your address, call us right away to avoid losing coverage.

  • Call L.A. Care at 1-833-522-3767 (TTY: 711) — open 24 hours a day, 7 days a week, including holidays
  • Mail or fax us your address form
  • Update your address with Social Security:

What Is “Unlawfully Present”?

Medicare rules say you must have legal permission to live in the U.S. If you’re undocumented or your visa has expired, you may be “unlawfully present” under the law.

What Happens Then?

  • You cannot get Medicare, Medicare Advantage, or drug plans
  • You’ll need to find other coverage until your status is fixed
