Attend our Member Orientation and receive a Healthy Rewards Card valued at $10! L.A. Care Medicare Plus (HMO D-SNP) members only who attend the L.A. Care 2024 Member Orientation are eligible to receive (1) Health Rewards card (limit one per year).
